Student Athlete Medical Document Checklist
- Health Care Provider Physical Exam: First Year student athletes must have a physical within 6 months of the start date in order to participate in athletics. Returning student athletes do not need a repeat physical. These exams are NOT provided on campus. Physical Examination Form
- Pre-Participation Exam: Each year student athletes will need to complete a medical history questionnaire online using SportsWare. This must be completed by August 1st. Upon arrival on campus, the student will meet at a scheduled time with an athletic trainer for medical clearance prior to participation in any practices/games/team testing activities.
- Insurance Cards: Each year athletes must upload front and back documentation of their health insurance cards online using SportsWare. This must be completed by August 1st.
- Sickle Cell Verification: Required to be completed for your first year of athletics. Does not need to be completed yearly.
- SWAY Testing: Instructions on how to complete SWAY testing will be provided before the start of the season.
Insurance Information / BMI Information
Thomas College does not bill for services performed by its Athletic Trainers. MaineGeneral Medical Center as well as other healthcare providers will bill the patient's insurance for their services. In most cases the student athlete's regular health insurance policies will be the primary insurance coverage. After this primary insurance coverage has been applied, if the injury occurred during an activity sponsored by Thomas College Athletics, Thomas's additional sports insurance policy will provide secondary coverage.
Primary Insurance: All medical services (except those provided by Thomas's Athletic Training staff) will be billed to the student's personal/primary health insurance plan. If the injury did not occur during an activity sponsored by Thomas College Athletic Department, the student and student's family will be responsible for copayments, deductibles, and any other out-of-pocket expenses. Given this, it is advisable for students to speak with their primary insurance company to gain a full understanding of what is covered, for all health needs arising outside of the athletic season.
Secondary Insurance:
As a benefit to student-athletes, Thomas College purchases an excess sports accident insurance policy through Bob McCloskey Insurance and BMI Benefits, LLC. This policy provides coverage when medical claims are incurred as the result of a covered injury while participating in collegiate athletics.
For varsity student-athletes, the Thomas College sports accident insurance policy has a $1,000 deductible (per injury) and provides up to $90,000 of coverage (per injury) after the primary insurance has paid its portion toward the claim. Additional coverage is automatically provided through the NCAA should the $90,000 maximum be reached.
For club and JV student-athletes, the insurance program includes a $1,000 deductible (per injury) and provides $25,000 in base accident medical expense coverage per injury. Once this limit is reached, catastrophic coverage applies, which includes a $25,000 deductible (satisfied by the base coverage) and provides up to $5,000,000 in coverage, with a 10-year benefit period. It is important to note that use of catastrophic coverage is rare, as most claims do not exceed the base coverage limit.
The Thomas College sports accident insurance policy does NOT cover non-sports-related conditions under any circumstances.

Physical Therapy Services
Thomas College Sports Medicine partners with MaineGeneral Medical Center to provide on campus physical therapy services. These services require a formal PT referral. All PT services are billed by MaineGeneral through the student's primary insurance and when appropriate will also be billed through Thomas College's secondary athletic insurance.
